For the fourth time this international competition for students of many medical professions will take a place during Annual Meeting of SESAM 2017 in Paris/France.

During this event winners of national medical simulation contests from many different countries will have a chance to compete against each other in challenging and very interesting simulated clinical scenarios.

Teams are expected to be composed of current students of many medical professions, like medicine, nursing, paramedics etc. They will have to manage not only high-fidelity human patient simulators but also some standardised patients (SPs). Each team will be expected to take history, perform a full physical examination and introduce an appropriate treatment to their patients. Apart from this communication, leadership, teamwork and crisis resources management will be evaluated by a panel of an international judges.

Cardiac Care Trainer System Features 

The Ambu Cardiac Care Trainer has a special intubation head that allows trainees to practice the oral intubation using modern airway management devices like ET tubes, Laryngeal mask and Combitube™. Realistic defibrillation up to 400 joule can be taught and practiced. The Defib Trainer W offers comes with 3-4 lead ECG. The CPR Software is part of the wireless system in the Defib Trainer W. The CPR Software analyses and displays the performance done on the manikin in real time. In addition the CPR Software can display and control the ECG. The virtual ECG provides 26 rhythms and 2 artefacts and can automatically detect defibrillation. The Ambu Cardiac Care Trainer W does not need cables and wires. Which means it can be placed in realistic surroundings away from computers and power supplies. The EYE Examination Simulator is an innovative trainer for fundus examination, designed to allow examination of eyegrounds with the physician"s own ophthalmoscope. Various cases can be set up for trainees using combinations of choice of slides, depth and pupil diameter. Soft and supple material allows hands-on simulation of real examination procedures, such as raising the eyelid.

Features Suitable for training with all types of direct ophthalmoscopes. The lens-equipped eyeball units reproduce the visual axis close to that of the human eye providing a realistic eye-ground view. When the instrument is used improperly, the ocular fundus will not be observed clearly. Pupil diameter can be changed. The depth of the fundus slides can be set in 3 steps, showing differences between hyperopic, normal and myopic views. Red reflex can be seen. 10 cases of fundus slides are provided for training in common eye disease identification. Its “brain” is ARM 9 processor which with the help of a build-in digital camera captures everything that was written and stores it on a memory card. Digital pen enables its users to easily create, retain and automatically digitize notes of any kind. Digital pen is fully programmable which allows not only data storage but also interactions between the user and the computer.Digital pen scans a special code that is printed on a paper. Each pen stroke activates a CCD camera which takes digital snapshots of the dot pattern at a rate of between 50 and 100 images per second. Each snapshot contains enough data to enable the pen’s image microprocessor to determine the exact position of the digital pen as well as what, where and when something was written. Having captured the coordinates from the paper, the digital pen provides key data about the handwriting context which allows determining what information will follow (e.g. a form filling situation). In addition, every digital pen has a unique pen ID which includes not only the details of what was written but also who wrote it, even if the text was written only on one page. All the data is then retained in the pen’s internal memory and if necessary transferred to the computer via Bluetooth or USB port. The digital pen alone would be worthless, if it were not for the dotted paper. All forms of conventional paper can be converted into digital paper by printing an appropriate dot pattern (a 600 dpi laser printer is sufficient). Every small area of the pattern has a unique combination of little, grey dots in different positions. When the user writes on a digital paper, the digital pen reads and retains the position of the dots and obtains the information about the location of the text. LABORATORY RESEARCH INNOVATION EMERGENCY MEDICAL SERVICES As part of the Built, Natural and Medical Research Centre, University of Rzeszów Innovative'' were designed and prepared research labs within LABORATORY RESEARCH INNOVATION Emergency Medical Services   The Laboratory for Innovative Research in Emergency Medical Services is:   1. Laboratory of innovative methods of simulation in emergency - SimMan 3G Completely wireless simulator with a build-in advanced Drug Recognition System. It registers the amount, speed and type of drug automatically and applies the appropriate physiological responses.  Simulated spontaneous breathing, bilateral and unilateral chest rise and fall, CO2 exhalation, normal and abnormal breath sounds. Unilateral, bilateral & lobar breath sounds, heart sounds and bowel sounds. ECG rhythm monitoring and 12 lead ECG display, defibrillation, cardio version and pacing. Advanced Video System Advanced video system simplifies the delivery and presentation of scenario debriefing by providing instructors with a tool to review all student assessments, therapies, patient interactions, critical thinking skills, and communication skills. Automatically integrates data from video cameras, a microphone, and patient monitoring with the event log creating a debriefing file that is easy to review with students, manage and share.   2. Laboratory measurement of resuscitation skills training – ALS Simulator (3 pcs) The Advanced Life Support Simulator is a realistic interactive training manikin for simulating a wide range of advanced lifesaving skills in medical emergencies. The simulator responds to clinical intervention, instructor control and comprehensive pre-programmed scenarios for effective diagnostics and treatment. With spontaneous respirations, airway control, voice, sounds, ECG, blood pressure, IV injections and many other clinical features, the ALS. The simulator is the fully functional emergency care simulator.   SimJunior Advanced SimJunior is a realistic, full-body 6 years old pediatric simulator which helps to learn all needed skills in pediatric care like tachycardia, asthma, anaphylactic shock, allergic reaction.  Automatic debriefing based on the event log synchronized with video pictures provides immediate, detailed feedback on performance and optimizes the simulation as an educational tool.   Defibrylator Philips HeartStart MRX Light, portable monitor/defibrillator is working in four mode: monitor, manual defibrillator, aed and pacemaker. Pulse Oximetry (SpO2), Non-Invasive Blood Pressure (NBP) and Capnography (ETCO2) are also available.   Post-traumatic shock laboratory In February 2014 tender proceedings for following equipment: respirator, infusion pump, bed, basic vitalsim monitor will be initiated.   All simulation laboratories are equipped in audio-video system what enables recording simulation sessions. This records, the physiological parameters from simulator and student’s performance are combined by computer system. Such prepared session gives full picture of simulation and can be played and reviewed during the debriefing. The center is equipped with advanced patient simulators (2 adult, 1 baby, 1 infant,  1 birth and 1 newborn .) In the center there are debriefing rooms, if necessary, schedule analysis can also be performed directly in the simulation room or other rooms. In addition to the facilities for the conduct of advanced simulation sessions center provides four classrooms, designed primarily to exercise manual skills, where less advanced phantoms and training models are used i.e. manikins to learn advanced rescue procedures, dummies for driving cardiopulmonary resuscitation, models limbs to learn punctures, intramedullary injections, models to study spinal anesthesia , intubation models for learning, cricothyrotomy, pneumothorax decompression and drainage of the pleural cavity, models of childbirth, to study the urinary tract catheterization kits and a small surgery . In the self testing rooms students and staff of IPRM can independently learn and test their skills in CPR AED, ALS, ITLS, ILS. In the future it will be possible to introduce the teaching with use of standardized patients or conducting objective structured clinical examinations (OSCEs). Next Generation Internet, NGI Poznan meeting with European Commision 08.06.2017

Place of meeting: PCSS Poznań, Jana Pawła II 10, nowe centrum 11-15:30 W czwartek 08.06.2017 odbyło się spotkanie z bardzo miłymi otwartymi przedstawicielami Komisji Europejeskiej w temacie Next Genaration Internet NGI lub jakkolwiek by nazwać przyszłą infrastrukturę informatyczną, którą Europa chce utworzyć w najbliższej kilkuletniej przyszłości. Przedstawiciele, osoby, trzeba podkreślić były nieprzeciętnie miłe i radosne. Po krótkim wstępie do tematu NGI trwającym około pół godziny, gdzie prowadzącymi byli: GeorgiosTselentis i Nicole Muessingmann (European Commision), zaprezentowało się przy okazji kilka firm i urząd miasta Poznania. Później odbyły się  ponad godzinne (3x 20 min.) zamienne warsztaty w trzech grupach z osobami wspierającymi prace komisji: Reserch: Nicole Muessingmann, (European Commision) Civil Society: Maria Chiara Campodonico (Martel Inovate) Startups: Richard Stevens (IDC) Na szczególną uwagę zasługują warsztaty gdyż to właśnie dzięki nim zostały uwolnione pokłady rozmaitych pomysłów jak NGI "ugryźć". Wszyscy uczestnicy brali udział w kazdym warsztacie po 20 minut i wyrażali swoje pomysły warte uuwzględnienia w pracach European Commision nad NGI: 1. Important Technologies and themes for NGI 2. Who are the most important stackeholders 3. What are the most propable businnesmodels Pause i Lunch. Podczas przerw serwowano ciasteczka i placki oraz liczne mieszane z mięsem i nabiałem kanapki, herbatę kawę i wodę w butelkach. Po za pociętymi plasterkami cytryny do herbaty nie podano soków owocowych ani nie uwzględniono uczestników o wegańskich czy witariańskich nawykach. Brak jakiejkolwiek nieprzetworzonej żywności. W świetle już wieloletnich wyników badań naukowych czy zeszłorocznej zmiany piramidy odżywiania IŻiŻ jest to wielkie zaniedbanie i świadczy o tendencyjności organizatora imprezy - z pewnością niezamierzonej.  W perspektywie spotkania ta niezamierzona tendencyjność pokazuje jak mocno jesteśmy jako społeczeństwo od wpływami kultury, tradycji i nawyków żywieniowych co prawdopodobnie przenosi się na sposób myślenia i tworzące się koncepcje w temacie NGI - odległe od statystyk, faktów i oczekiwanej rzeczywistości informatycznej. Po spotkaniu organizator zapewnił uczestnikom niezapomniane wrażenia podczas prezentacji laboratoriów i serwerowni PCSS Poznań. --------------------------------------------------------------------------------- Komentarz Z. Gólski, uczestnik spotkania z 08.06.2017 Podejcie KE do tematu uważam, że było uszyte na miarę. Otwartość prowadzonych warsztatów umożliwiała zabranie głosu każdemu uczestnikowi. Fakt organizacji spotkania w PCSS pokrywa się z usytuowaniem projektu w przestrzeni gospodarki ale z oczywistych powodów może być postrzegany jako jednostronny. Technologie IT są tworzone w podobnych PCSS ośrodkach ale większościowymi odbiorcami NGI będą zwykli ludzie. Dlatego osobiście mam nadzieję, że takie spotkania w temacie NGI są i będą przez KE również organizowane w najróżnieszych środowiskach. Next Generation Internet - background Temat NGI to temat rzeka. To wyzwanie dla człowieka, którego zainicjowanie postawiła sobie Europa. Jednocześnie wszyscy chyba czujemy, że czas coś z Internetem zrobić. Oczekujemy z chęcią jakiś zmian. Niepokoją nas tez różne zjawiska do czego Internet bywa używany. Minęło 20 lat Internetu a 90% użytkownikom wciąż internet służy miłemu spędzaniu czasu i co ważne również nauce i poszerzaniu świadomości - co się dzieje mimowolnie. Szczególnie skok w świadomości ludzkości jest głównym owocem ostatnich dwóch dekad Internetu. Może jeszcze nie wiele wiemy na co miałoby się to praktycznie przydac, słyszeliśmy jednak o wielu dziwnych sprawach, które nas nie zajmują ale w innych częściach globu mają swoje nawet wielotysięczne rzesze zwolenników. Podsumowując krótko, z perspektywy NGI, dzisiejszy użytkownik Internetu jest jego czytelnikiem. Jeśli już coś tworzy "trwałego" to albo jest zatrudniony albo jest specjalistą, dla którego ze środowiska sieci uczynił źródło utrzymania. Serwisy społecznościowe mają ogromny potencjał twórczy jednak praktycznie mamy tam do czynienia z przemiałem treści, informacji, często o zabarwieniu emocjonalnym i znaczeniu osobistym. To co się jeszcze nie dzieje to tworzenie ruchu i treści kontentu merytorycznego (mający istotny wpływ na wzrost świadomości, wiedzy i gospodarki) na poziomie udziału przynajmniej 50% wszystkich użytkowników sieci. NGI czy first human tool Istnieją na świecie środki i metody jak sprostać problemom. Są osoby, wolontariusze i specjaliści , którzy mogliby się spotkać online i wymienić poglądy, informacje i zasoby. Ale brakuje wspólnego narzędzia komunikacji. Dzisiaj jest tak, że kiedy już się spotkają wreszcie, to dyskutują, wymieniają informacje i znajdują rozwiązania (nawet małej części problemu) ale z powodu braku podstawowego narzędzia nie mogą określić (dla innych) przynajmniej kierunku dalszych poczynań. Mamy wiedzę w internecie ale nie posiadamy narzędzia, które ją uwiarygodni, pomoże w jej zrozumieniu, dostosuje do naszych możliwości oraz wesprze w jej zastosowaniu. Na koniec narzędzie powinno utworzyć raport aby jego ponowne użycie było o stopień efektywniejsze. Dlatego bardziej brakuje narzędzia niż środowiska jakim miałoby być NGI. Od czego zacząć? Podejćie KE jest w porządku, wyraźnie wspomniano, że projekt jest otwarty, nie mają określonych celów ani od czego zaczną, przynajmniej tak oficjalnie. Pytanie jednak ma głębsze znaczenie. W ciągu 18 lat pracy nad podobną strukturą doszedłem do wniosku, że po pierwsze należy TO zacząć. Rozpocząć formalnie projekt z realnymi ludźmi. Gdziekolwiek i z kim kolwiek zachowjuąc całkowitą przejrzystość, otwartość i być gotowym na skalowalność projektu. Na początek może to być istniejący projekt o odpowiednich walorach (jest ich w internecie wiele) lub niewielka aplikacja wykonana w ciągu 2-3 miesięcy. Cel czy kierunek NGI? Z pewnością powinno się badać i planować kierunek rozwoju NGI. Określanie celów to stawianie barier, które kiedyś przyszłe pokolenia będą ograniczały. Określenie kierunku dotyczy spraw o które powinniśmy zadbać i takich które powinnismy świadomie unikać, nie rozwijać. Przykładem jest większy nacisk na planowanie przyszłości i teraźniejsze działanie NGI oraz niskie zaangażowanie w badania i analizę historycznych, politycznych, czy ekonomicznych faktów. Jaki kierunek? Pierwszy kierunek wyznaczają aktualne problemy czy niewiedza społeczna chociaż badania naukowe już dawno udowoniły stan faktyczny lub aktualny a ciągle pokutują przyzwyczajenia, nawyki, tradycja, kultura, które hamują postęp. Następnym kierunkiem są oczekiwania postępu w związku z aktualnymi potrzebami, tymi zwykłymi sprawami w urzędach, rodzinach, szkołach, transporcie, komunikacji grup zawodowych itd. Jaki pęd zmian? wszystkie działania i narzędzia wynikające z NGI powinny zachować cechę zrównoważonego rozwóju społeczeństwa. Jest to żelazna reguła gwarantująca minimum strat różnych grup społecznych w związku z wprowadzanymi zmianami przez inne grupy czy środowiska.  cdn... --- Autorem tekstu w tym bloku jest Ziemowit Gólski, kóry w latach 1998-2016 intensywnie i na miarę swoich ograniczonych możliwości zajmował się "tematem NGI" rozwijając z ponad 100 osób taką koncepcję. Od platformy wiertniczej, pracy w USA i Niemczech, poprzez własne działalności, pracę w reklamie, drukarniach i innych firmach, korporacji i na uczelni, razem około 30 zawodów i więcej miejsc pracy. In Poland, in the academic year 2013/2014, was for the first time introduced a course of medical studies, after the completion of which students do not have to complete a medical internship in such a dimension as before. Hence, the examinations entitling future medical doctors to perform their professional duties will be taken just in Medical Simulation Laboratories which imitate small hospitals with dozens of rooms that have to be built by the university. There are, among others: Emergency Rooms, ICU, neonatal rooms, operating theatres - all fully equipped with real equipment. In 2013, the Ministry of Science covered the cost of architectural projects of Simulation Laboratories for 11 universities of medical sciences, and guaranteed for each university PLN 23 million (Equivalent of 5 milion Euro) on infrastructure and equipment. For example, the Simulation Centre in Poznan will have three floors and nearly 3,000 m2. The similar situation is observed at other universities. In total, the cost of the equipment in simulation laboratories in Poland which will be implemented over the next 2-3 years will amount to PLN 300 million (Equivalent of 75 milion Euro). Now, in 2016 we all universities startet to build their Simulations Centres. We hope in April-Mai we started buyin equipment, manikins for Simulation Centres. Every year representatives of Medical Simulation industry meet at professional conferences connected with the fair. In 2014, the organizer of the Anniversary SESAM Meeting for Medical Simulation was the University of Medical Sciences in Poznan. For the first time the conference was held in the country of the old Eastern Bloc.

Sound pick-up and sound transmission have been thoroughly reworked and added to a new overall concept, which sustains any comparisons with regard to its acoustic performance.  Auscultation is significantly disturbed by interfering side noise. Therefore during the construction phase of the cardiophon2.0 and the duplex® 2.0 special attention has been paid to the design of the binaural and the ear-tips. The contact pressure of the binaural can be adjusted individually; the ear-tips are pivotable and made of extra-soft material. Altogether the ear canal is perfectly sealed against external side noise. Introduction Various medical specialty organizations and the Agency for Healthcare Research and Quality (AHRQ)1 have advocated using ultrasonography to guide central venous cannulation. It is surprising then, that very few instructional models have been described to teach this technique. Consequently, we developed a model to teach ultrasound-guided central venous access. This paper presents a recipe for an ultrasonographic model or "phantom" that is easily made, inexpensive and simulates vessel cannulation extremely well. Methods The phantom is formed using water, unflavored gelatin, sugar-free Metamucil, latex tubes and a rectangular Pyrex glass cake pan (17 × 27 × 5 cm). Preparation of the container includes cleaning the inner surface and spraying it with a small amount of a non-stick product. To make the phantom mixture, first determine the volume of water needed to fill the desired container one-third full. Boil the water and then gradually whisk in 3 packets of gelatin for every 250 mL of water. Continue to whisk or stir the mixture until the gelatin is completely dissolved. Next, add the Metamucil in portions of 1 tablespoon for every 250 mL of water. Whisk until it is completely dissolved. Any remaining clumps of gelatin and Metamucil should be removed at this time. Lastly, pour this mixture into the container and put it in the refrigerator for 1-2 hours or until it is firm. This concoction now forms the base layer of the mould. While the base layer of the mould is congealing, select the latex tubes (Penrose drains) that will be used to simulate the vessels. One-half inch diameter tubes work well to simulate the neck and femoral vessels and one-quarter inch diameter tubes appear like brachial vessels. Tie the latex tubes at one end and fill them with water, being careful to minimize the amount of trapped air. The volume of water in each tube determines whether it will be used to simulate an artery or a vein. For example, tubes that have less water will compress easily and therefore will appear like veins. After the desired volume of water is placed in the latex tube, tie off the open end. After the base layer of the mould is firm, place the latex tubes on top of it. A second aliquot of the gelatin-Metamucil mixture (enough to fill one-third of the container's volume, as described above) is poured onto the base layer, with enough volume to surround and cover the latex drains. This will form the middle layer. Chill the mould again until firm. Lastly, prepare another gelatin-Metamucil mixture and pour it on top of the middle layer until the container is filled to the point that the latex drains are no longer visible. Chill the mould a third time until it is firm and then remove it from the Pyrex container. At this point the phantom is ready for use. Conclusion Ultrasound-guided central venous access is a skill that emergency physicians will need to add to their armamentarium. Prior educational tools are limited, and this paper describes a phantom that is easily made, inexpensive and simulates ultrasound guidance of vessel cannulation extremely well. A full environment simulator is similar to flight simulators used to train pilots. The pilot is immersed in a complete replica of the cockpit environment.
